Juvenile Justice Care Coordinator

Position Title: Juvenile Justice Care Coordinator
Location: Cincinnati, OH
Status: Full-Time
Reports to: Family Care Manager
Works closely with: Juvenile Justice Manager, Youth Development Manager, Care Coordination Team, Coaching Team


Position summary: The Juvenile Justice Care Coordinator offers holistic, trauma competent, relational care for youth and families to keep families united and growing together toward dependence on Jesus, interdependence in community, and economic independence and sustainability.

Primary responsibilities:

  • Maintain caseload of youth and families involved in the Juvenile Justice system
  • Walk youth and families through the intake process and implement an assessment system to address current opportunities and barriers in the home
  • Individualize and deepen care for youth and families in the home by helping create and maintain child development plans
  • Teach regulation skills and help youth and families build individualized calming and regulation plans
  • Utilize Social Work modalities to help youth and families find their voice, build goals, mobilize their strengths, and overcome obstacles to their sustainability
  • Coach caregivers toward healing, connection, and interdependence as a family
  • Collaborate with an interdisciplinary team to provide holistic care for youth and families
  • Lead in crisis management, assess safety concerns, and problem solve as needed
  • Collaborate with local agencies involved with youth and families and make referrals to additional local agencies as needed
  • Connect youth and families to resources as needed
  • Develop, lead and participate in communities of belonging as assigned
  • Maintain documentation in the Back2Back Care Database
  • Attend appropriate staff meetings and trainings
  • Develop and maintain a team of ministry partners that fully fund the position
  • Assist with other duties as assigned

Required education and experience:

  • Bachelor’s degree or higher in Social Work (licensure preferred)
  • Training in Trauma Competent Care
  • Experience working with children and families

Required skills and abilities:

  • Clear understanding of and commitment to the Back2Back vision and mission

  • Operates with basic competency in Gmail, Google Calendar, Google Drive, Google Docs, Google Sheets, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.
  • Access to reliable transportation for providing community-based care
  • Understanding and practice with theories of trauma and attachment
  • Knowledge of resources available to support youth and families in Cincinnati
  • Cultural knowledge and sensitivity
