Youth Care Coordinator

Position Title: Youth Care Coordinator
Location: Cincinnati, OH
Status: Full-Time
Reports to: Back2Back Cincinnati Youth Care Coordination Manager
Works closely with: Back2Back Cincinnati Team, Partner Organizations
Position summary: Utilize the 5-Point Child Development Plan, Trauma Competent Care Principles, and Social Work Modalities to create a safe space where students can use their voice, process their feelings, and learn skills that will empower them towards dependence on Jesus, interdependence in community, and economic independence and sustainability. 

Primary responsibilities:

  • Maintain a caseload of up to 20 youth with at least 2 meetings per youth per month
  • Conduct Intake Assessment (using Child Status Index, Adverse Childhood Experiences Survey, and Conor-Davidson Resilience Scale)
  • Facilitate the creation of a holistic Personal Development Plan for each youth
  • Utilize Social Work Modalities to give youth a voice, mobilize their strengths, and equip them to overcome obstacles to their success and sustainability
  • Connect children and families with resources, services, and opportunities
  • Collaborate with an Interdisciplinary Team to provide deep holistic care for youth
  • Maintain case records in the Back2Back Care Database
  • Aid with crisis response plans and problem solving as needed
  • Attend appropriate staff meetings and trainings
  • Develop and maintain a team of ministry partners that fully fund the position
  • Other duties as assigned

Required education and experience:

  • Bachelor’s degree or higher in Social Work (licensure preferred)
  • Training in Trauma Competent Care
  • Experience working with children or families
