Position: Ministry Case Worker

Position Description: Create, develop and implement programs to strengthen the organization in continuous improvement to meet the identified needs of the kids, teens, and adults that the ministry serves, specifically to Urban Cincinnati. Using the 5 Point Child Development Plan and the Child Status Index to offer valuable support and assistance to families in need who are a part of Urban Cincinnati.

Primary Relationships: Reports to Back2Back Urban Cincinnati Director

Responsibilities:

  • Design and implement student and family admission system for the Back2Back Urban Cincinnati Program
    • Admission formats
    • Interviews with candidates/families and elaborate diagnose
    • Back2Back requirements
    • Child Protection Services requirements
    • Programs admission calendar
  • Coordinate relationship with Urban Cincinnati and Back2Back
  • Maintain a caseload of 3-5 clients-families who are a part of Urban Cincinnati
    • Conduct regular in-home assessments (using Child Status Index, Risk Assessment, Environmental Scans)
    • Visit each family once a week
    • Help set visions and goals for each child/family
  • Collaborate with social worker, psychologists, teacher, health professionals, spiritual leaders, etc. to determine action plans and review goal progress
  • Aid with emergencies and problem solving
  • Attend appropriate staff meetings and trainings
  • Work with other staff to help plan activities (field trips, health visits, tutors, etc)
  • Maintain records
  • Connect children and families with resources, services, and opportunities

Qualifications:

  • Mature Christian (see doctrinal questionnaire)
  • University degree (preferably in Social Work, Psychology, or Community Development)
  • Clear understanding of the vision and mission of Back2Back
  • Access to transportation to enable regular in-home visits
  • Experience working with children or families
  • Trauma Competent Care (TCC) trained
